After receiving your Phase 2 compliance notification, you may submit a request through ResDAC to obtain Medicare data. Please include "QE Request" in the subject line of the email.
Establish a new Data Use Agreement (DUA)
To request data for the first time, you must submit the following documents to establish a DUA with CMS.
Prepare drafts (no signatures) of the documents and email to resdac@umn.edu. ResDAC will assist with obtaining a cost invoice, finalizing documents, and submitting to CMS.
Add data to a Qualified Entity DUA
When you need to add new files or years to the existing DUA, it is called an amendment. You must submit the following documents to amend your DUA.
Prepare drafts (no signatures) of the documents and email to resdac@umn.edu. ResDAC will assist with obtaining a cost invoice, finalizing documents, and submitting to CMS.
Make changes to individuals listed on a Qualified Entity DUA
QEs work directly with the QECP CMS Contact, Kari Gaare, to make changes to individuals listed on the QE DUA. All CMS requests must include Kari Gaare and QECP DUA for review and approval. Only the requester or data custodian listed on the DUA (if they include the requester on the email) may initiate a contact change.
CMS must be notified if there is a change in your organization's requester, custodian, or custodian's contact information.
Required form:
The current requester listed on the DUA drafts an email with the following information:
- Email Subject: "Request to [change Requester/update requester contact information] on DUA [your DUA number]"
- Email Attachment: The signed, completed Signature Addendum (with new/updated requester contact information)
The requester listed on the DUA sends the email to the CMS QE Program Contacts and CC's the qecp_dua@index_analytics.com mailbox.
