The values in this field depend on the Provider Number System/Nomenclature used:
If PRVDR_NUM_SYS equals “1” (State-specific Medicaid Provider ID), then this value is the ANSI state numeric code (e.g., 04,17,33,48).
If PRVDR_NUM_SYS equals “2” (NPI) or “3” (Medicare), this value is “CMS.”
If PRVDR_NUM_SYS equals “4” (NCPDP ID) then this value is “NCPDP”
If PRVDR_NUM_SYS equals “5” (Federal Tax ID), then this value is “IRS”.
If PRVDR_NUM_SYS equals “6” (State Tax ID), then this value is the name of the state’s taxation division (e.g., DOR, OK TAX).
