Navigation case (NC) standardized nine-digit unique navigation case identifier.

| Alphanumeric character string that contains intelligence (e.g., 323006639 is the 6,639th navigation case for the AHC bridge organization B23). | The first digit is always 3, then the 2nd and 3rd digits are the numeric portion of the bridge organization identifier (i.e., the “B” from the bridge organization ID is replaced with a “3”). The remaining 6 digits are sequential numbers representing the navigation cases within the bridge organization. |
For each navigation-eligible beneficiary who opted in for navigation, the bridge organization opened a “navigation case,” assigned a case ID. The bridge organization provided navigation services for up to 12 months.
This ID is specific to the AHC Model and does not link to any other CMS data file or system.
