Navigation case (NC) action plan complete indicator. This field indicates whether a navigator developed a personalized action plan for addressing HRSNs with the beneficiary who opted in for navigation services.

This was not a required field until October 2019, so if a beneficiary has a missing value, it does not necessarily mean the navigator did not work with the beneficiary to complete a personalized action plan. It is possible the navigator and beneficiary completed the personalized action plan but did not document it in the AHC data system.
